Private School Applications
To ensure our timely response, application requests must be turned into us by Friday, December 3rd. We cannot guarantee that requests will be completed if your request is submitted after our deadline. It will be at the teacher’s discretion if they have time available.
Electronic Applications (examples: Ravenna)
For private schools that use an online application system, please have the records request or “Common Records Request” sent to the Registrar at email@example.com.
PSIS Administrator Forms should be sent to your student’s grade-level counselor.

Our request(s) for a teacher recommendation should be addressed to the teacher from whom you require a recommendation.
Please provide a standard business sized envelope (addressed to the applicant school with adequate postage to cover at least two ounces.) This is required for each school you are applying.
It is also courteous to ask the teachers in advance (via e-mail) if they would be willing to fill out the forms, before dropping the forms off in their mailbox at the school.
Please submit requests early. The teachers and staff will process recommendations as time permits during their normal work schedule.
The Main Office does not coordinate Teacher Recommendations. It is the student’s responsibility to work with teachers for the recommendations. If you have any more questions, please contact the Toni Morgan at email@example.com
Paper (offline) Applications
For private schools that use paper applications please drop off a signed application form along with a private school addressed envelope with two (2) stamps to Main Office by Friday, December 3rd. We will need a prepared envelope for each school if there are multiple schools.
What will be included in School Records:
- Progress Report (1stQuarter – this year)
- Middle School Academic History
- Standardized testing
- Attendance Report (this year)
- Incident Report

Eckstein office staff does not handle teacher recommendations.
Your student must ask their chosen teacher in person BEFORE you submit that teacher?s email address. If teachers are not asked by the student to fill out the recommendation information they will not comply. For offline teacher recommendations students must Hand Deliver ?teacher? evaluation to staff of choice with a self-addressed envelope with one (1) stamp. This is mailed separately by the teacher.

Eckstein office staff does not handle teacher recommendations. Students must personally ask his/her teacher for the recommendation then hand deliver the Teacher Evaluation Form to that teacher with a private school addressed envelope with one (1) stamp. This is mailed separately by the teacher.

Note to Parent: In most cases, schools are responsible for requesting records for incoming transfer students.
If a school requires that parents/guardians provide a student’s records, reach out to the student’s most current school. Include as much of the information below as you can
How other schools/districts should request records:
When submitting your request, please make sure to include all of the necessary information that will help us fill the request in a timely manner:
- Student’s Information
- Full Name
- Date of Birth
- Student’s current grade level
- Requesting School’s name and address
- Requester’s name, title, email, phone number, fax number
- List of Records being requested
- Private Schools should also include a signed release form
Note: Seattle Public Schools (SPS) does not use the OSPI Student Records eXchange.

- Special Education Records are managed in IEPOnline (IEPO). If a school does not subscribe to IEPO, Special Education Records requests should be sent to firstname.lastname@example.org.
- Please email Advlearn@seattleschools.org for Advanced Learning records.
- If the student in question has significant health concerns, please have the family work with:
- Their medical provider to share information as needed and/or
- The nurse at the student’s former school directly if there is information managed by the school that needs to be shared.
- If a certificate of immunization is needed, families should create an account on MyIR.net to access the immunization records for their student(s).
- Copies of Birth Certificates will not be provided.
