FinalForms for Athletics and Extracurricular Activities

How to Use FinalForms

Eckstein Middle School is using FinalForms for all athletic forms and many extracurricular activity forms. You only need to create an account for your student once. This account will be available to your student for the remainder of their educational career with SPS.

Parents/Guardians will need to create an account first through the Parent Portal.

Once you are in, you will need to add your student.

Add a student

You will need to register your student with a non-SPS email for your student. This can be another adult email, your student’s non-SPS email, or create an email via Gmail specifically for FinalForms.

Accessing the Appropriate Forms

You will add forms by clicking on the Add Sport/Activity Link. The Eagle Night contract is under Extracurricular Activities.

This will pull up a page where you can find the Sport/Activity you need. Eagle Night Dance contracts are under Extracurricular Activities. If you don’t see it in your list. Click on Update sports/activities and Choose Coed Dance Contract (Eagle Night.)

Completing the Forms

For all forms, Athletic & Extracurricular Activities that need to be completed by a parent/guardian, must be completed by the adult first. Then the student will get an email prompting them to complete their portion of the form.

The student will then follow the link in the email to create their own account in FinalForms that they access through the Student Portal.
